Register as a New Patient

To register at Elmwood Family Doctors as a new patient, please follow these 3 simple steps:

1. Check to see if you live within the practice catchment area

Please be aware that if you are close to the boundary you may need to check with the practice manager. Only patients that live within the practice catchment area will be accepted as a new patient.

2. Submit your registration

You can now register with us online. Just fill in the quick online form to start the process.

Paper forms are still available at our reception desks if you need one.

Alternatively you can download and complete a Patient Registration Form.  Once completed, please return the form along with 2 proofs of identity; one must be photographic and one must show your current address. Utility bills or bank statements must be no more than 3 months old.

3. Book your New Patient Health Check 

If you have registered online, a receptionist will contact you to arrange this appointment. If you hand in a paper form, the receptionist will book your health check with you at the front desk.

Please bring a urine sample with you to the appointment. Please note that you will need to see a GP / Nurse if you are on repeat medication. It can take a while to get your full medical records from your previous GP, hence it is best if you make an appointment for an initial assessment and we can then make sure you get all your medicines and checks on time.

Temporary Patient Registrations

You can register as a temporary resident with a GP surgery for up to 3 months. This can be helpful if you’re away from home but do not want to change your home GP surgery.

To register you’ll need to fill out a temporary services form (GMS3), available from our reception desk.

After 3 months, you’ll have to reapply to register as a temporary resident or become a permanent patient.

Non-English speakers

Our website can be translated to many different languages using the google translate tool at the bottom left of each page.

Language interpreters can be requested with advanced notice.
