What is a Long Term Condition Review?
A Long Term Condition review is a type of health check for people with Long Term Conditions. These conditions include:
- Diabetes
- High blood pressure
- Kidney disease
- Heart disease
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Rheumatoid arthritis
- Learning disabilities
- Dementia
- Severe mental health problems
If you have one of these conditions then you will be invited for an annual review or more frequently if necessary. It is vital that you attend all of your appointments so that we can ensure your treatments are still appropriate and safe.
What will happen?
A member of the reception team will contact you around your birth month, either by SMS appointment invitation link or phone call, to invite you to attend your annual review.
The first part of this is with a healthcare assistant. At this appointment we will take a blood sample, check your blood pressure, height, weight and update other health data.
They will also perform a foot & leg check (if diabetic) and discuss your eating regime and lifestyle.
Your results will be reviewed by a member of our clinical team. Depending on your results you may not need a further review and we will update your medications accordingly.
If we need to discuss your results and treatment with you, you will be invited to book a follow up appointment with either a nurse, pharmacist or advanced practitioner. We will contact you by SMS appointment invitation link or phone call to book this the week after your initial appointment with the healthcare assistant.
